    Anti-Müllerian Hormone Concentrations in Women of Different Reproductive Age and the Chances of IVF Outcome: A Paradigm Shift is needed

    Abstract: „The study of Anti-Müllerian Hormone (AMH) has garnered considerable attention due to its critical implications in assessing and understanding both female and male fertility potential. Traditionally, AMH is recognized for its pivotal role in evaluating ovarian reserve and is a cornerstone in reproductive health assessments for women. The aim of this study was to challenge the traditional interpretation of AMH as a standalone predictor of IVF success. Through a retrospective analysis of 600 patients undergoing ICSI, we reveal that women with low AMH levels, traditionally classified as poor responders, can achieve unexpectedly high oocyte numbers, blastocyst formation, and pregnancy rates. This highlights the limitations of using AMH alone to predict IVF outcomes. Our findings advocate the importance of integrating additional factors, such as follicle-stimulating hormone (FSH), and the need for a more individualized approach to fertility treatment planning.”(…)

    Beyond Spherical: Unveiling the Significance of Oval Blastocyst Morphology on Euploidy and Implantation Success

    Abstract: „The selection of the most suitable embryo, based on the morphology and shape, for embryo transfer is a critical aspect of the in vitro fertilization (IVF) process, as its precision can significantly enhance the overall effectiveness of IVF and contribute to a healthy birth. This study aimed to compare the chromosomal status and implantation potential of oval-shaped blastocysts versus normal-shaped blastocysts on day 5 post-ICSI (intracytoplasmic sperm injection). Initially, the frequency of oval blastocysts was assessed by analyzing 1328 embryos from 610 ICSI cycles. Subsequently, 80 patients undergoing ICSI and PGT-A (preimplantation genetic testing for aneuploidy), who had both normal and oval blastocysts in the same cycle, were selected to evaluate the euploid rate relative to blastocyst morphology. Finally, the implantation outcomes of fresh embryo transfers involving oval and normal-shaped blastocysts, neither of which had undergone PGT-A, were analyzed. Half of the blastocysts from each group were transferred after assisted hatching (AH), and the other half were transferred without AH. Blastocyst shape does not appear to correlate with an increased risk of aneuploidy but does influence hatching ability. Following AH, the implantation potential of elongated blastocysts is equivalent to that of normally shaped blastocysts, suggesting AH is beneficial for oval embryos. Consequently, the transfer of oval blastocysts is considered as safe and effective as the transfer of normally shaped embryos.”(…)

    FOS and Other Genes Linked to Oxidative Stress Are Upregulated in Women Suffering from Endometriosis and Might Negatively Impact the Oocyte Developmental Competence During ICSI

    Abstract: „Purpose: We aim to elucidate the impact of endometriomas on oocyte developmental competence and identify genetic alterations that may influence ICSI success rates in women of different ages. Methods: In this retrospective study, two groups of patients have been analyzed: 108 women with a history of severe endometriosis (stages III and IV), who underwent OPU and ICSI after endometriosis surgery, and a control group, built out of 108 women with no endometriosis history. The Turku Endometriosis Database was used for differential gene expression analysis. Results: The number of aspirated oocytes (endometrioma group: 2.75 ± 0.32 vs. control 10.32 ± 0.54) was significantly (p < 0.0001) lower in patients with endometrioma excision, and the number of generated blastocysts was significantly (p < 0.0001) lower when compared to patients without endometriosis (endometrioma group: 1.09 ± 0.13 vs. control group: 3.76 ± 0.23). Additionally, AMH levels were significantly lower (p < 0.001) in the endometriosis-affected group below the age of 35 years (1.65 ± 0.15) than in the unaffected control group (2.99 ± 0.27) within the same age group. The mRNA expression of the following genes related to oxidative stress was significantly (p < 0.05) upregulated in the endometriosis group compared with healthy controls: DES, P2RX1, FOS, SRF, RASD2, SLITRK3, and TEAD3. Gene Ontology and pathway enrichment analyses demonstrated a significant (p < 0.0001) enrichment of oxidative stress–associated biological processes and signaling pathways, including responses to reactive oxygen species and hypoxia, HIF-1, MAPK, PI3K–Akt, NF-κB, and apoptotic signaling, supporting oxidative stress as a central molecular mechanism underlying the observed transcriptomic alterations. Conclusions: In conclusion, this retrospective study provides a holistic perspective on the impact of endometrioma and its oxidative stress effect on oocyte developmental rates and ICSI outcomes across different age groups.”(…)

    Novel multi-omic biomarkers to combat oocyte and ovarian aging

    Abstract: „Understanding oocyte and ovarian aging has become critically important, as trends in family planning evolve, with many women choosing to have children later in life. The ovary, a crucial organ in female reproduction, is particularly susceptible to age-related changes and is one of the organs that exhibit functional deterioration most distinctly with age. The aging of female reproductive systems also affects longevity and various health outcomes. A better understanding of both oocyte and ovarian aging will lay the cornerstone to elucidate the phenomenon of longevity in women. Here, clinical data from 400 women of various ages undergoing intracytoplasmic sperm injection (ICSI) have been analyzed, including Anti-Müllerian Hormone (AMH) and Follicle-Stimulating Hormone (FSH) levels, the number of recovered oocytes, blastocyst rates, pregnancy rates, and live birth rates. Our analyses revealed significant differences in the aforementioned rates between patients of young and advanced age. For the biomarker analysis, we further utilised a novel predictive performance of age-associated gene expression signatures for oocyte aging, demonstrating its potential to provide molecular-level insights into oocyte quality over time. By analyzing RNA sequencing data generated from human oocytes of different ages, a genome-wide landscape of age-associated gene expression has been described. Additionally, metabolome profiling has been performed on young and reproductively aged mice, serving as a model for human ovaries. Changes in metabolites of the murine ovaries during aging have been recorded. In conjunction with traditional biomarkers, multiomics data represent a transformative approach in reproductive health, and they may offer personalised risk assessments and interventions to mitigate age-related fertility decline in women. Our metabolome profiling provides a valuable resource for elucidating the metabolomic basis of ovarian aging. Our findings offer novel insights into systemic shifts associated with oocyte and ovarian aging. This integrated approach may unlock new avenues for fertility preservation, ovarian rejuvenation, and assisted reproduction.”(…)

    Reproductive aging, preimplantation genetic testing for aneuploidy, and the diameter of blastocysts: does size matter?

    Abstract: „There is no doubt that maternal aging, also known as reproductive aging, can contribute to the increased rates of aneuploidy observed in blastocysts generated from women of advanced age who undergo in vitro fertilization (IVF). Additionally, the hatching process of the blastocyst, which is crucial for successful implantation, may be impaired in aneuploid embryos. Aneuploid embryos often exhibit abnormal cell division and chromosomal distribution, which can lead to disruptions in the hatching process. Due to ethical restrictions, preimplantation genetic testing for aneuploidy (PGT-A) is unavailable in all countries. Therefore, our retrospective study of 502 couples who underwent intracytoplasmic sperm injection (ICSI) aimed to elucidate if embryonic features, such as the ability to hatch and embryonic diameter, could be a reliable estimator for the success rate after embryo transfer, especially for women aged 26–45 years, and for IVF clinics which do not have access to PGT-A. The small hatching blastocysts (Bl. 5) group had a significant (p < 0.001) higher percentage of euploid embryos (≤35 Y- 73%, >35Y- 51%) compared to large (Bl. 4) counterparts (≤35 Y-58%, >35 Y- 38%). In patients aged 34-38 years, we detected 10% more euploid blastocysts in the hatching group than the expanding ones, which was a significant difference (p < 0.05). In conclusion, when selecting non-PGT-A tested embryos for embryo transfer (ET) or frozen embryo transfer (FET), a small hatching blastocyst seems to be a better choice than a large expanded one, especially for advanced-age patients for whom the risk of aneuploidy is higher.”(…)
